Terms:

Gift Certificates may be redeemed at CNY Fertility for any serviced billed by CNY Fertility, excluding egg, sperm, embryo, or tissue storage. Gift certificates have no cash value and can not be redeemed for cash, check, or anything other than treatment credit at CNY Fertility. Gift certificates are non-refundable. Gift certificates expire 10 years from the date of purchase. Once redeemed, funds will be applied first to any outstanding balance.  After purchase, a payment to CNY Fertility will appear on your card statement. It may take UP TO 5 BUSINESS DAYS for our team to retrieve the payment and post it to your patient portal.

How to Redeem a gift certificate/treatment credit:

The purchaser should receive an email confirmation with an 8-digit receipt number within a few minutes of purchase in the inbox indicated during the checkout process. This 8-digit receipt number serves as the gift certificate code and should be shared only with the intended recipient.  This code must be presented to a member of the CNY Fertility staff to be redeemed.
