Terms:

Gift Certificates may be redeemed and applied to current open balances or future services at CNY Fertility, excluding egg, sperm, embryo, or tissue storage. Gift certificates have no cash value and can not be redeemed for cash, check, or anything other than treatment credit at CNY Fertility. Gift certificates are non-refundable. Gift certificates must be redeemed to your account in full. Once redeemed, you may not transfer any unused portion to another person. Gift certificates expire 10 years from the date of purchase. Once redeemed, funds will be applied first to any outstanding balance.

If you purchase multiple gift certificates in a single transaction, they will be issued and treated as one combined certificate. If you wish to receive separate certificates, for example, to gift to different individuals, you must complete separate transactions at the time of purchase.

How to Redeem a gift certificate/treatment credit:

The purchaser should receive an email confirmation with an 8-digit receipt number within a few minutes of purchase in the inbox indicated during the checkout process. This 8-digit receipt number serves as the gift certificate code and should be shared only with the intended recipient. To redeem your gift certificate, please email it to: costs@cnyfertility.com and please let us know what balance/services you wish to apply the funds to. Funds will be applied to your account within 24-48 hours.
