Gift Certificate Terms:

Gift certificate purchases must be made online only. Gift certificates may be redeemed and applied to most current open balances or future services at CNY Fertility, excluding egg, sperm, embryo, or tissue storage. Gift certificates have no cash value, are non-refundable, and cannot be redeemed for cash, check, or anything other than treatment credit at CNY Fertility. Gift certificates must be redeemed to the recipient’s account in full and are not transferable once redeemed. Gift certificates expire 10 years from the date of purchase, and once redeemed, funds will be applied first to any outstanding balance. If multiple gift certificates are purchased in a single transaction, they will be issued and treated as one combined certificate; to receive separate certificates for gifting to different individuals, separate transactions must be completed at the time of purchase. Gift certificate offers cannot be combined with any other promotion, discount, special pricing, or sale.

how to redeem Gift Certificate:

After purchase, the purchaser will receive an email confirmation containing an 8-digit receipt number, which serves as the gift certificate code and should be shared only with the intended recipient. To redeem a gift certificate, email the receipt number to costs@cnyfertility.com and indicate which balance or services you would like the funds applied to. Gift certificate funds are typically applied to the recipient’s account within 5 business days.

treatment with medication package Terms:

Treatment with Medication Packages are available for new purchases made online, in office, or by phone and must be paid in full at the time of purchase. Online purchases may take up to 5 business days to be reflected on the client account, and medication orders may take up to 5 business days to process. After purchase, clients will receive confirmation in their patient portal along with a financial agreement outlining full terms, conditions, exclusions, and instructions. Treatment with Medication Package discounts cannot be combined with any other promotion, discount, special pricing, or sale, and previously purchased gift certificates may not be used toward discounted packages. The offer is not valid on previously purchased packages. Treatment cycles using a discounted package must begin on or before June 30, 2026. If treatment does not begin by that date, the payment will convert to a credit that may be used toward any service or package at the then-current pricing.

how to redeem treatment with medication package:

After purchase, the package will be applied to the client’s account within 5 business days. Clients will receive confirmation in their patient portal along with a financial agreement detailing full terms, conditions, exclusions, and next steps. To redeem the package, the client must schedule and begin their IVF or Egg Freezing treatment cycle on or before June 30, 2026, in accordance with their care team’s instructions.
