insurance plans, we now offer a per cycle discounted payment plan
and a multi cycle IVF Refund Plan, which help to make our treatments
more affordable and less of a financial burden to you.  |

IVF
PER CYCLE PLAN (2)
This interest free option was created to help our
non-insured patients pay for their treatments over a period of
one year. We do require a minimum payment prior to retrieval, and
the balance will be divided into twelve interest-free monthly payments.
On the 15th of every month, we will withdraw payment from your
debit or credit card. Your payment includes egg retrieval, ICSI, assisted hatching,
embryo transfer, cryopreservation, storage and transfer of frozen
embryos for up to one year. After one year, any additional frozen
transfers will be $1,000.00. These prices DO
NOT INCLUDE MEDICATIONS.

IVF REFUND
PLAN
The IVF Refund Plan was created for non-insured
IVF patients who meet the eligibility requirements (see below).
For one single up-front payment of $20,000 you are eligible for
up to 6 fresh IVF cycles and unlimited frozen cycles over a two
year period. Payment in full is required prior to starting your
cycle. If you deliver a baby within the six fresh IVF or frozen
cycles, you will not be eligible for any refund. However, if
you fail to deliver a baby after the 6 fresh IVF and all frozen
cycles, your payment will be refunded in full.
IVF Refund Plan Qualification Criteria:
- Females under the age of 38 at time of enrollment
- No insurance benefits for IVF
- Day 3 FSH <9 and Estradiol <100
- Have not failed more than three previous IVF cycles at any
fertility center
- The ability to generate an endometrium lining thickness of
7mm
- No hydrosalpinx or significant uterine fibroids (unless corrected
prior to treatment)
- Male partner’s semen analysis not indicative of azoospermia
or severe oligiospermia. Post-vasectomy patients may qualify,
but require a diagnostic aspiration.
What is included in the IVF Refund Plan:
- Retrieval of eggs
- Transfer of embryos
- All frozen embryos must be transferred prior to starting a
fresh cycle
- Intracytoplasmic Sperm Injection (ICSI)
- Assisted Hatching (AH)
- Cryopreservation and storage of embryos for up to one year
after last cycle (additional fees apply after the one year period)
What is excluded from the IVF Refund Plan(1):
- Follicular monitoring ultrasounds at our offices
- Follicular monitoring blood work at our offices
- Initial consultation and any pre-screening tests
- Surgeries
- Urology procedures
- Donor Sperm
- Medications (Approximately $3000 per cycle)
- Preimplantation Genetic Diagnosis (PGD)
- Intravenous Immunoglobulin (IVIG) therapy
- Pregnancy testing and monitoring
- Egg Donation (See Donor Refund Plan)
- Embryos may be transferred into a carrier for an additional
charge

PAYMENTS
All payments for fertility services are due in advance
of treatment.
IVF procedure payments are due as follows: Minimum of 50% down
at the time of the procedure. The remaining portion of your payment
may be financed, and monthly payments will be directly deducted
from your debit or credit card for 1 year, interested free. To view

MEDICATION
COST ESTIMATE
Medication costs vary for each patient. Some patients require higher doses than

you may contact them for at 1.877.750.7222. The approximate cost for IVF cycle
medication is $3000. |

INSURANCE
PLANS
Insurance benefits will vary with each patient
and insurance carrier. Prior to your initial consultation, our
office will contact your insurance company to verify infertility
benefits, and medication coverage. After your consultation you
will be informed of your benefits and any out-of-pocket expense
you may incur. If we do not participate with your insurance carrier,
we will bill them for you as a courtesy.

Your payment includes monitoring for oocyte donor (at our office),
monitoring for the recipient (at our office) until day of embryo transfer including up to two ultrasounds for the Mock Cycle, egg retrieval, embryo transfer, cryopreservation, storage, transfer
of frozen embryos within one year of retrieval date, and donor compensation.

EGG
DONOR REFUND PLAN

or frozen cycles, you will not be eligible for any refund.
However, if you do not deliver a baby after the 4 Donor cycles,
and all frozen cycles, your payment will be refunded in full.
Donor Refund Plan Qualification Criteria:
