JUSTINE TAYLOR, RN, BSN,
is our Nurse Manager and IVF Coordinator in our Latham office. She monitors patient stimulation cycles with ultrasound. She performs blood draws, patient education and intrauterine inseminations. She also oversees the daily operation of the Latham office and its staff. A graduate of Samaritan Hospital School of Nursing in Troy, NY and The State University of New York at Utica/Rome in Utica, New York, Justine has worked in OB/GYN nursing for 19 years, specializing in Reproductive Medicine since 1994.

Dear Lareina,
A beautiful day in the universe to all, and thank you for
allowing us to be involved in your lives. Over the last several years I have
been on a journey of life and learning.
Our entire office staff recently went to a retreat and part of the
experience was to do a daily intention. I
have been writing this daily and sending this out to my staff and recently
began sending out to friends, colleagues and am excited about sending them to everyone who is
interested. This has been a very powerful experience for me and I believe that
each and everyone of you would benefit from creating your own daily intention.
We each set the tone of our day and our lives. The daily intention is a great
way of reminding us to be positive, hopeful and persistent. I welcome each of
you to share and continue to be as positive as you can be.
The fertility journey is a difficult road to travel, but it
is the energy of the universe that gives us the ability to proceed. The goal
for each of us is to find the tools that allow us to travel this journey in the
most positive and hopeful way. Reading
books on self improvement such as the Power of Now and a New Earth by Eckhart
Tolle, is one powerful way of improving our thinking. In addition, yoga,
acupuncture, music and dance are all methods used to improve our lives. In
addition, CNY Fertility has a support group that meets monthly and an on-line
support message board, both are a great way of letting
go of negative energy and improving our lives.
Participation is the first part of the journey. Thinking
less and doing more is the second. None of us know the outcomes of the day. By
walking out the door each and every day we give ourselves the possibilities of
the joy and benefits we seek.
Remember to exercise the mind as we all do for the body.
Live life to the fullest and take the leap
of faith.

Male Factor
|

One of the contributing factors to a couple's fertility difficulties can be problems with sperm number or function.
Approximately 15-20% of infertility can be ascribed to male factor infertility. When sperm are evaluated, they are assessed for quantity, motility, and how normal they look. Results can appear normal, borderline, or severely abnormal. Severely abnormal semen analyses will often require in vitro fertilization with intracytoplasmic sperm injection (IVF-ICSI) to achieve pregnancy. Normal or borderline sperm may be able to generate a pregnancy, but at a reduced chance of success. An important item to remember is that a semen analysis does not measure male fertility; it measures sperm production. From those numbers, we try to draw connections between analyses and chances of pregnancy, but the correlation is not always exact.
If problems with sperm production are identified, urological evaluation may be suggested. A urologist can perform a focused history, physical exam, and laboratory assessment, that might identify a reason behind the male's difficulties. Many times, a hormonal or anatomic reason can be identified, which might be treated with medication or surgery.
Treatment for male factor generally involves increasing the number of normal, motile sperm having access to the eggs. This can be done with timed intercourse around the time of ovulation, intrauterine insemination, or IVF-ICSI. With intrauterine insemination, the seminal fluid is exchanged with neutral, hypoallergenic media, and the best-swimming sperm are collected after treatment in a centrifuge. Misshapen, immotile sperm are discarded, and the fraction of strong-swimming sperm is placed with a thin catheter into the woman's uterus. Conception will still rely on the sperm's ability to reach the egg, bind to its shell, and fertilize. If an insemination does not work, there is no way to tell where the process failed.
IVF with ICSI minimizes failure by leaving the least to chance. Individual normal, motile sperm are carefully placed into oocytes, giving the highest chance for fertilization. The fertilized eggs are placed directly into the uterine cavity, minimizing the chance for ectopic pregnancy.
Other recommendations for maximizing male fertility include eating a regular diet, and avoiding tobacco, alcohol, and illegal drugs. One multivitamin per day may also help. The elements most likely to impair sperm number or quality include chemotherapy, radiation therapy, and anabolic (muscle-building) steroids. Many times, male infertility can not be "fixed", but its effects can be minimized with intrauterine insemination or in vitro fertilization.

On Thursday, April 10, 2008, people from across the nation who care about family building issues will exercise their rights, and journey to Washington, DC, to meet with their Congressional leaders.
RESOLVE constituents will meet with staff members of their U.S. Senators and House Representatives to discuss issues they care about:
- Co-sponsorship of The Family Building Act of 2007 (HR2892), federal legislation mandating insurance coverage for infertility diagnosis and treatment. For more information on this legislation including a list of current co-sponsors, click here.
- Make permanent the adoption tax credit.
- Increased funding for medical research for the National Institutes of Health (NIH).
- Support the goals of RESOLVE's National Infertility Awareness Week, October 19-25, 2008.

Brad & Corey's StoryWe tried it all! IUI, shots, you name it and nothing was working. So, we moved on to IVF and I was surprised at how long it took. My embryos looked great, counts were high... so why did it take so long? Fresh, frozen, fresh, frozen... felt like forever! And the cost! WOW! Don & Laura's Story My husband and I had been trying for three years. The last year that we tried, we went to CNY Fertility. It is my belief that ovarian drilling, clomid treatments and my weight loss (20 pounds) helped us get pregnant! We are very grateful for all that Dr. Kiltz and his office did to help us. Now we just celebrated our son's first birthday.

John & Elizabeth's Story My husband and I were just trying to live out the, "American Dream". We were married and bought the perfect house to start our family. Everyone around us seemed to be having babies, we assumed we would be next. However, two miscarriages and five years later, we were still no closer to our dream. That's when we decided to take all our hopes and go to CNY Fertility Center. I still remember our first appointment with Dr. Kiltz. He looked at our chart, ran some tests that day, then said, " We are going to get you pregnant". The doctors and staff became our family as our fertility journey began. Six months later, it was decided IVF was our best option. We transferred three embryos, and found out on our second ultrasound that we were blessed with twins. The staff was so happy and supportive of us. They even referred me to a new amazing OB practice, after I had fears of returning to my old OB. On July 14, 2007 we finally became parents. Our girls were four weeks early, but so perfect. Meghann weighed 6 lb 10 oz. and 17 in long. Makenna was 6 lb. 3 oz and 18 in long. They came home with us five days later. We had to go by CNY on our way home from the hospital. So, we stopped to show the girls where it all started. The whole staff cried and cheered, it was like they had twins of their own. If I could have stayed and had the doctors at CNY deliver my babies, I would have. I love the center and all of the staff. How can you thank them enough for what they do for people everyday? I am reminded of my gratitude towards CNY each morning when I walk into my girls room and see these two smiling faces looking back at me.

patients pay for their treatments over a one-year
period. A minimum payment is required prior to
retrieval, and the balance is divided into twelve
interest-free monthly payments. On the 15th of every
month, payment is withdrawn from a

ICSI, assisted hatching, embryo transfer, cryopreservation,

one year. After one year, any additional frozen transfers are
