Clomid Citrate

Clomid citrate is commonly the first medication that is pre­scribed for patients who are unable to ovulate on their own with unspecified infertility in combination with insemina­tion. Women who have normal fallopian tubes, regular 28 day cycles, and partners with a normal sperm count, but are still experiencing infertility, are termed as having 'unspecified' infertility. The pituitary gland regulates the amount of FSH (follicle stimulating hormone) and LH (leutenizing hormone) in the system. These two hormones play a key role in ovulation. The levels of these hormones determine when and how many eggs are developed and re­leased. LH is responsible for the further maturation and re­lease of the egg(s).

 

Clomid citrate is an anti-estrogen medication. This means that it tricks the pituitary gland into thinking that the levels of estrogen in the body are low, causing the pituitary gland to secrete additional FSH and LH. This increase of FSH and LH stimulates the development of the follicles which contain the egg(s). Clomid citrate is taken as a pill, and is generally  pre­scribed as one (50mg) pill each day for 5 days in the beginning of the menstrual cycle (days 3-7). A mature follicle is usually found around day 12 of the cycle. If ovulation does not occur, the medication can be changed to reflect the patient's needs.  Ultrasound is the best way to determine the number and maturity of the follicles. Ovulation predictor kits can be used to measure if there has been a surge of LH mid-cycle, indicating ovulation has occurred. Ovula­tion occurs about 24-28 hours after the detection of the LH surge in the urine.  Once ovulation has occurred, natural or artificial insemination is performed in an attempt to fertilize the egg(s) that have been produced.

The amount that a woman weighs plays an important role in the level of estrogen in her system. Fatty tissue excretes addi­tional estrogen into a woman's system. Too much estrogen or too little estrogen can throw your body into a whirlwind causing irregular periods or anovulation (the inability to ovu­late). Twelve percent of infertility cases are due to weighing too little or too much. Rectifying and then maintaining weight standards, including a healthy diet and regular exer­cise and not smoking can help to increase your chances of conceiving. Hysterosalpingogram
(His-tur-oh-sal-ping-go-gram or HSG)

Although tricky to pronounce, a Hysterosalpingogram (HSG), is a simple in-office procedure, which determines whether the fallopian tubes are open, and assesses the presence and condition of fibroids or polyps in the uterine cavity. Very low radiation x-rays are used via fluoroscope, while iodine contrast dye is gently injected into the uterus and fallopian tubes. The procedure takes roughly 20 minutes.
 
The female patient lies on an exam table under the fluoroscope. A speculum is placed in the vagina, and a catheter is used to administer the contrast dye through the cervical opening, into the uterus. Once the dye is injected, the fluoroscope is used to determine whether the dye fills and spills out of the fallopian tubes, indicating the fallopian tubes are open.

An ultrasound called a hysterosonogram, may be performed at the time of the HSG providing an enhanced evaluation of the uterine cavity. A Hysterosonogram can provide information which an HSG alone could miss. 

An HSG is performed for diagnostic purposes; however there has been some evidence that an HSG may increase fertility in the months following the procedure. There are very few risks associated with this procedure, but it is important to tell your doctor if you are allergic to iodine, IV contrast dyes or seafood. There is an alternative for patients with these allergies.

Patients have rated an HSG as mildly to moderately uncomfortable, experiencing discomfort similar to menstrual cramps. A non steroidal, such as Advil (Ibuprofen) taken ½ hour prior to the procedure can minimize this discomfort. After the procedure the patient can resume their normal daily routines. Some spotting may occur, but heavy bleeding should be reported to our staff immediately.

CNY Fertility Center offers HSG onsite, alleviating the need to go to a hospital for this procedure.  Following the procedure your doctor will be able to diagnose tubal or uterine factor infertility, and a treatment protocol can be established.

 
 
A Message from Dr. Kiltz

Much has happened in the 10+ years since I started CNY Fertility Center.

Beginning in 1997, I opened the areas first comprehensive fertility center at Community General hospital in a 1500 square feet space and with no employees. My goal was to provide quality care at a more affordable price. I quickly added much needed help (staff) and  grew  in the first year completing  about 25 IVF cycles. We soon had the areas  first IVF delivery, frozen embryo delivery and donor egg delivery. In 2001 we moved to our new home at 195 Intrepid Lane, a state of the art center with over 7000 square feet, with an up-to-date IVF lab and accredited surgery center.

We now have over 60 staff members between Syracuse and our newest facility in Albany. Caring not only for patients from Central New York, but as far away as Australia, England, Lebanon and Canada. Despite our growth, we continue to focus on quality individualized care.

Our self pay IVF costs are the lowest in the industry while still maintaining statistics above national averages. The New York State Grant program has allowed hundreds of patients to undergo IVF at significant cost savings.

Our 10 year anniversary saw over 3000 patients, family members and babies visit the Syracuse Gifford Zoo, helping us celebrate a huge milestone.

We recently opened CNY Healing Arts, an integrative practice incorporating traditional Chinese medicine with western medicine. Along with acupuncture, massage, yoga and nutrition these modalities will aid in improving personal health, well being and hopefully improve outcomes. The
Circle of Hope and Fertile Friends support groups have been providing needed guidance and care for several years now and have proven to help outcomes. I highly recommend the use of these resources.

We hope that these additions will continue to assist us all in the fertility journey as well as maintain a positive view on life, despite all of the stressors that each of us go through. It often seems difficult in the journey of life, but the exercise of the mind is as important as the exercise of the  body in maintaining an optimistic and positive outlook.

Series - Discovering Your De-stress Style
Join us for a one hour, 4 week session to learn different types of relaxation and stress reduction tools.

De-stress techniques are not a one size fits all, you must discover what suits you best according to your learning style, personality, personal preferences and lifestyle. We will learn easy chair yoga, guided visualization, progressive muscle relaxation exercises and breathing techniques. All these techniques are easy to learn and suitable for non-meditators and experienced meditators alike. There is something for everyone here, have fun finding what works for you.

Instructor Connie Heagerty is a mind/body de-stress educator who is certified in Yoga, Reiki, Hypnosis, and is a trained meditation instructor. She is a member of the American Board of Hypnotherapy, The International Association of Mind-Body Professionals and The Yoga Network.

Classes are $40 for 4 weeks. Classes will be held on the following Wednesday's from 4:00 PM - 5:00 PM.
